Two conditions may lead to hospital admission
- Elective: You may have been advised by your physician to make the admission.
- Emergency: A patient who is admitted to the emergency department due to a serious illness or life-threatening condition.
Admission Checklist
Advance registrations/admissions are a viable option. Registration forms will ask for the following information:
- Name and address
- Health insurance details
- An emergency contact
- Treatment consent
- Consent to release information about insurance companies mentioned
- Signing the agreement and consenting to payments due
You will need these elements for a seamless experience in healthcare
- You can find your hospital records (old/recent), test results, and a list of medications you have taken.
- List of allergies to medication, if any.
- Recent hospitalizations or medical summaries, if applicable.
- You can get your insurance card.
- Change of clothing, toiletries, and nightwear; eyeglasses, hearing aids, dentures, and any regular prescription medications.
