Our patient’s medical records are kept in electronic format and are available upon request. Each patient’s medical record is a unique document that chronicles the medical treatments given in the past and the present, as well as the health of the patient’s past and the present. It also includes treatment plans for future health care. Our health care system consists of the delivery of your medical records. Processes and procedures are in place to ensure that you receive continuous and coordinated care.
- The medical report will include the following: a patient’s medical history, pathological results, radiology, laboratory, and operative reports.
- Lab reports and other studies are available.
- All discharge summaries for inpatients, emergency department reports, health nursing reports, and physiotherapy reports are provided.
- The MRD number of each patient can be used to retrieve all their medical records.
- Only authorized personnel have access to all medical records.
- We ask that you contact the radiology department if you require images or photos of radiology. They will have the necessary data.
